What is Support Coordination?
NDIS support coordination involves optimising an NDIS participant’s plan by connecting them with the right people and services that meet their needs. To achieve this, an NDIS support coordinator works closely with a participant to help them fully understand their plan and then assist with its implementation.
At Empowrd, our support coordinators are committed to helping NDIS participants in Port Lincoln and Eyre Peninsula find and access the supports and services they need to achieve greater independence and resilience. We provide assistance with identifying, understanding and implementing the goals in their NDIS plan. We also help participants develop independent living skills and build personal capacity.
